A PT Harmoni Panca Utama (HPU) dump truck operator died after jumping from a truck whose brakes failed on July 10 2025 in the mining area of Berau Regency, East Kalimantan. In a panicked condition, the victim gave information via radio with the code “brake loss” or brake failure before jumping out. Footage from the vehicle behind shows the moment the victim fell and his head hit the ground. He was treated for two days at Dr Abdul Rivai Regional Hospital, but ultimately died.

Ironically, the accident was only reported to the East Kalimantan Manpower and Transmigration Service (Disnakertrans) in the Berau region on July 29 2025, or 19 days after the incident. The Labor Inspection Coordinator, Saban, regretted the delay in this reporting, because according to regulations, incidents with fatalities must be reported within a maximum of 3×24 hours by the company’s K3 Expert.
PT HPU management admitted that they were late in reporting because they were focusing on internal affairs. However, the Manpower and Transmigration Department suspects that the report was only submitted after information about the accident spread on social media. Saban even believes that there are strong indications of efforts to cover up the incident from the public.
Disnakertrans ensures that investigations continue to be thorough, including into allegations of violations of reporting procedures and work safety negligence. Until now, PT HPU management has not provided an official statement regarding the technical causes or the results of the internal investigation.
